Free Assessment Planning Template: Build Your School or District Roadmap

A strong assessment readiness strategy requires more than marking test dates on a calendar. Schools also need to plan what happens before and after each assessment, from instructional checkpoints and data reviews to intervention, communication, and follow-up.

As Dr. Mya Asberry, Director of Instructional Services at MasteryPrep, said during a recent webinar, “Your assessment plan is, in fact, a journey. And whenever we’re going on that journey, we need a roadmap.”

  • Building an Assessment Planning Strategy

A roadmap should connect testing windows with the work that surrounds them—when scores will be available, when teams will review results, when intervention will begin, and when students will be reassessed. That planning can span everything from national college readiness assessments like the ACT and SAT to state-specific End-of-Course (EOC) exams.

This approach can create greater visibility across teams. Rather than relying on separate documents or informal planning, you can rely on one shared roadmap to understand who’s responsible for what and how each step connects to the broader readiness strategy.

The goal is to create a more intentional approach that spans the entire year, rather than treating preparation as a standalone moment or a series of disconnected events. These templates are designed to help you identify gaps early and coordinate support.

  • The Assessment Calendar Template: Getting Started

We offer two versions of our free assessment planning template, providing greater flexibility in how you organize your plans. Both are customizable and fully editable so stakeholders can easily map key milestones, assign ownership, track progress, and customize accordingly.

  • The Calendar Grid Edition offers a traditional month-by-month calendar view for teams that prefer to see their assessment plan laid out visually. 
  • The Log Table Grid Edition organizes the same planning process in a sortable, filterable table. 

Both versions include a completed August example, along with blank monthly tabs for the rest of the school year.
